Admissions Scheduler

Location: Care Dimensions
Reports To: Director of Admissions

Position Summary

The Admissions Scheduler plays a vital role in ensuring patient referrals and admissions are handled efficiently, accurately, and compassionately. Working under the direction of the Director of Admissions, this position coordinates referral intake, verifies insurance benefits, schedules admissions activities, maintains patient records, and serves as a key point of contact for patients, families, referral sources, and healthcare providers. The Admissions Scheduler helps ensure a seamless admission experience while supporting Care Dimensions' commitment to exceptional end-of-life care.

Qualifications

  • High school diploma required; Associate's degree in a related field preferred.
  • One (1) to three (3) years of experience in a healthcare, medical office, or related environment; or an equivalent combination of education and experience.
  • Proficiency with Microsoft Office applications, including Outlook, Excel, PowerPoint, and healthcare-related software systems and databases.
  • Professional, courteous, and compassionate telephone manner.
  • Strong organizational skills and attention to detail.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Excellent verbal and written communication skills.
  • Ability to interact effectively with patients, families, referral sources, and interdisciplinary team members.

Essential Responsibilities

  • Gather and document patient demographic, clinical, and insurance information for prospective admissions.
  • Verify insurance coverage, hospice benefits, and obtain required authorizations for services.
  • Coordinate and schedule registration, liaison, and admission visits within established timeframes.
  • Maintain current knowledge of hospice reimbursement options, eligibility requirements, and insurance processes.
  • Manage and maintain accurate databases of prospective and admitted patients, generating reports as needed.
  • Respond to inquiries regarding hospice services and provide information to patients, families, healthcare professionals, and referral sources.
  • Prepare and distribute informational packets to prospective patients and contribute to ongoing improvements of admission materials.
  • Maintain referral logs and compile referral statistics and reports as requested.
  • Ensure patient census information remains accurate and current within Care Dimensions systems.
  • Coordinate the completion, copying, and distribution of admission paperwork and required documentation.
  • Foster effective communication and collaboration among Admissions, Clinical, and Management teams.
  • Build and maintain positive relationships with referral sources, healthcare partners, and community agencies.
  • Provide outstanding customer service by responding promptly and professionally to internal and external customer needs.
  • Demonstrate reliability, responsiveness, compassion, courtesy, and professionalism in all interactions.
  • Perform additional duties and projects as assigned.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package – including employer-sponsored health insurance, paid time-off, tuition reimbursement, student loan paydown, supplemental insurance options and more – which adds to the company’s generous total rewards package.

Pay Range$23—$26 USD

Benefits are offered to employees that are scheduled to work 20+ hours/week, which include a generous earned time (vacation days) program, tuition reimbursement, scholarship programs, student loan paydown program, two retirement plans, in addition to medical/dental/vision/life/disability insurance, and so much more!
